Below you will find a list of the online forms used with the NCLOR.

• Account Request Form – This form is used to request an account.
• Contact Information Form – This form can be used to contact the NCLOR staff.
• Issue Reporting Form – This form is where you can report any issues or problems with in the repository.
• Lost Password Form – Complete this form if you have an account but can not remember your password or username.
• Resource Suggestion Form – Complete this form if you have a series of resources you would like batch imported into the repository.
